ADVANCED PRACTICE NURSING CARE OF ADULTS WITH CARDIAC DISEASE
M. McCarthy and P. Komnenić
Arizona State University, College of Nursing, Tempe, Arizona, USA

In the United States the cost of health care has risen dramatically over the past twenty years.Expenditures are estimated in the trillions of dollars. This increased cost of health care will be accelerated by the increased prevalence of chronic disease, the aging of the population, increase in life expectancy, an increase is disability, and new medical technology. As a result, advanced practice nurses are assuming more health care responsibilities in a variety of settings. This trend is most evident in the area of cardiovascular care. Advanced practice nurses, master's prepared registered nurses who function in expanded capacities, function in the context of society, and by means of a social contract, are accountable to the public they serve.Advanced practice nurses are responsible professionally and ethically to uphold standards of practice in order to serve the public optimally.Nurses who are educated to care for adults with cardiovascular health problems are especially suited to work with clients across the health continuum. Primary health promotion as well as secondary and tertiary care can be provided to clients with problems ranging from those at risk for developing cardiac disease to those who have established and serious acute chronic heart disease. Advanced practice nurses are able to provide care to clients in either collaborative or autonomous practice situations.Advanced practice nurses may choose to work in a partnership arrangement with physicians and other providers or may practice independently.Regardless of practice circumstances, research overwhelmingly supports the contention that care provided by advanced practice nurses is of the highest quality and associated with positive patient outcomes. In providing care to adults with cardiovascular disease, advanced practice nurses assume many different roles such as teacher, counselor, direct caregiver, consultant and researcher. This paper will illustrate specific examples of each of these expanded roles using the Neumann System Model as a framework for practice.
